The following information is being shared on behalf of the Niagara Ontario Health Team-Équipe Santé Ontario Niagara (NOHT-ÉSON).

We are proud to share the NOHT-ÉSON’s latest media release regarding a significant investment in local client care.

The Let’s Go Home (LEGHO) program has received an additional $125,000 in one-time funding from the Ministry of Health. This investment allows the program to expand its capacity through March 31, 2026, helping more older adults return home safely after a hospital stay.

How the Program Works

LEGHO is designed for eligible adults aged 65 and older who have stable, non-emergency conditions. The program provides a six-week bundle of flexible, client-led services, which can include:

  • Meals on Wheels to support food security
  • Transportation and homemaking services
  • Safety at Home assessments to reduce the risk of falls
  • Respite care and specialized navigation for those with cognitive impairments

With the current cold and flu season leading to an increase in referrals, this funding bump is expected to support at least 50 additional clients. By focusing on a “Home First” approach, the program ensures patients recover in familiar surroundings while allowing hospitals to focus on acute clinical needs.
